If I could add anything to the list, I would say that another reason is a lack of consistency.

In other words, failing to maintain contact with your subscribers and, ultimately, them forgetting who you are.

All of our inboxes fill up pretty quickly, these days, so it's not hard for people to get confused (IMO).
